• What you enter in our forms: your name, phone number, email, the procedure you’re asking about, and your message.
• Records of your calls, texts and emails with our office. Calls may be recorded for quality and training, and we tell you at the start of the call.
• Basic technical information our web host needs to deliver pages, such as your IP address and browser type. We set one cookie that remembers your language choice. We don’t use advertising or analytics trackers on this site.

How we use it

• To answer your request, schedule your appointments and remind you of them, and coordinate your care.
• To send you marketing messages, only if you agreed to them separately.
• To run and secure our website and phone systems, and to meet our legal obligations.

Who we share it with

We don’t sell your information. We share it only with service providers that help us operate (our phone, texting, email, patient-contact and website providers), under agreements that limit their use to serving us, and when the law requires it.

Health information

Once you become our patient, the health information in your medical record is also protected by our Notice of Privacy Practices under HIPAA, which you receive at your first visit. Please don’t use our website form for emergencies. Call 911.

Your choices

You can stop texts at any time by replying STOP, and marketing email with its unsubscribe link. To ask what information we hold about you, correct it, or ask us to delete it, contact us below. Some records we must keep by law.

Security

We use reasonable safeguards to protect your information, but no system is completely secure.

Children

This website isn’t directed to children under 13, and we don’t knowingly collect their information.
